Hi and welcome to Roots Integrative Medicine! We are thrilled you are interested in becoming a patient at Roots. Please listen to the following voice recording (option 3 when you call our phone number 803-266-9920; transcript below), as it contains a lot of helpful information around our office policies and procedures, particularly for those interested in joining our practice. Who We Are
Roots is a full-functioning primary care clinic for ages 1 and up that takes an integrative medicine approach, meaning we blend traditional medicine with holistic, root-cause care.
As a primary care office, we offer many traditional primary care services, such as sick visits, physicals, pap smears, medication management and referrals if needed, EKGs, joint injections, minor procedures, in-house labs through Quest, and more. While we do not bill insurance for office visits, we do bill your labs through your insurance, though coverage varies and we cannot guarantee what your insurance will or will not cover.
Our biggest difference is that we spend much more time with patients — typically 60-minute visits — and go much deeper into your health. We prioritize nutrition, exercise, sleep, hormones, nervous system health, gut and immune health, and encourage our patients to be an active participant in their health!”
Care Model
Because our providers spend significant time before, during, and after visits reviewing your history, labs, and creating plans, we do not bill insurance for office visits. Insurance just does not reimburse for longer, in-depth integrative visits.
We can send you an invoice or superbill that you can personally submit to your insurance for possible reimbursement; as we have had many patients have success with this, although not guaranteed.
You are welcome to use your HSA or FSA account to pay for these visits.
Payment for all visits and services is due at time of service.
All of our prices are on our website rootsim.org, but to put simply, we do have a membership based program
The primary care membership is $40 per month, which provides access to visits with our practitioners, plus discounts on all supplements and services that we offer.
We also offer a services-only membership for $30 per month, which allows access and discounts to all supplements and services outside of office visits with our practitioners.
All pricing is transparent and listed on our website at rootsim.org. Your recurring membership is set up at your new patient visit and withdrawals on the 15th of every month.
Effective February 1st, 2026:
The cost of your New Patient Visit is $299. This is a 60 minute appointment and prior to meeting with you, your practitioner will spend a good amount of time reviewing your in-depth health history questionnaire that you will complete at least 48 hours prior to your appointment, as they start to plan 1-2 days before your appointment.
We often describe the first visit like dumping out a 1,000-piece puzzle — we are just beginning to see your full health picture and do not rely on quick fixes.
Membership may be canceled at any time; a $50 cancellation fee applies if canceled within the first 12 months
To schedule a new patient appointment, we require a nonrefundable $75 booking fee that will go towards your total new patient cost. Once scheduled, you will receive consents and questionnaires by email along with instructions to activate your Charm patient portal. Please make sure to click on the first link in your email to set up the Charm patient portal prior to completing the questionnaires.
Please complete all forms thoroughly — plan for this to take about one hour — and submit them at least 48 hours before your appointment; the sooner you do this, the better, as sometimes the link to activate your Charm portal expires if you wait too long. This is extremely important. If you have trouble with activating your portal or filling out your questionnaires, please contact us immediately (and at least 48 hours prior to your appointment so we have time to troubleshoot!) If not completed on time, your appointment may need to be rescheduled and a fee may be charged, which can be up to the full amount of your new patient appointment. Please also note that a no show to your new patient appointment will result in the full new patient appointment fee charged.
We do send you a text reminder 48 hours prior to appointments as well.
After your initial visit, your provider typically orders fasting morning labs with a follow-up visit 2–3 weeks later. Advanced functional testing (such as comprehensive gut microbiome, hormone, toxin, and food sensitivity testing) is available and you can discuss this further during the appointment with your practitioner.
We also offer many other services, such as IV nutrient therapy, infrared sauna, ozone therapies, peptide therapy (such as semaglutide and tirzepatide injections), health coaching, low dose allergen therapy, PEMF therapy, microneedling with PRP, PRP joint injections, and more. Details, pricing, and FAQs are available at rootsim.org and we encourage you to read through it prior to your first visit with us, as we try to be as transparent as possible.
Follow-up visits range from $175–$249 depending on complexity and are scheduled for ~60 minutes.
Sick visits are $60 for routine and $110 for complex, with the majority of the time same-day visits being offered.
When scheduling your appointment, we please ask that you arrive 20–30 minutes early so that we can get you checked in, the nursing staff can get you roomed, vital signs obtained, chart reviewed and teed up for your practitioner by the time of your office visit. For example, if you have a 10am appointment, please arrive no later than 9:40am.
We have a 10 minute grace period, although this will cut into your total appointment time, which is why it is imperative to arrive early.
We have a 24 hour cancellation charge of $75, which applies to office visits and health coaching visits, as we are prepping, planning, and already precharting for your visit 1-2 days before your actual visit.
We also have a no show fee of the full cost of a new patient appointment ($299), the cost of your scheduled follow up (either $175 or $249), the cost of your sick visit (either $60 or $110), the cost of your health coaching appointment ($150 for members), and a $50 no show fee for IV therapies.

Integrative Medicine is an approach that combines traditional medical treatments with alternative therapies, focusing on treating the whole person. It emphasizes the importance of a partnership between the patient and healthcare provider to address not only symptoms but also underlying factors contributing to health issues. Integrative medicine considers lifestyle, nutrition, and holistic practices to promote overall well-being. This approach aims to provide personalized, patient-centered care that may include a range of evidence-based traditional and alternative therapies.
Roots Integrative Medicine is a cash-based, membership-based, direct primary care clinic. This model gives you more time with your provider, access to selective discounted supplements and other alternative healthcare modalities, and a more personalized healthcare experience that is not available through traditional insurance-based models. Unfortunately, traditional health insurance does not currently reimburse for prolonged patient visits and the integrative care that makes Roots special.
You can, however, use debit/credit cards and HSA dollars towards payments. We are also happy to bill your insurance for lab work and other additional testing. The only real monetary difference between Roots vs. a typical practice is payment for appointments with the provider, as well as select procedures. See comparison table below for more details on this subject.
Yes! For ages 1+, Roots can be your primary care provider. It is important to note, especially for pediatric patients, that prior to joining the practice, there will be a consent acknowledging that vaccines may be ordered by the Provider to be administered at a local pharmacy or SC Health Department, but will not be currently administered at Roots. If you refuse a vaccine for your child, you may be given a refusal form and informed consent to sign.
Also, while there will be an option for After Hours messaging, this will not be a 24-hour service. Any urgent messages sent after the assigned After Hours time ends (see times below), will not be answered until the following day and you should seek emergency care for yourself or your child during that time if needed.

Yes! After 4 immediate family members join the membership at $30/month per person, the 5th+ immediate family members get a discounted rate of $15/month per person. Immediate family members are limited to 2 adults (ages 18+) per family. Roots office staff will use their best discretion when honoring the family rate.
Yes! There will be a messaging line through ChARM Connect for after hours care. You can use ChARM Connect through an app on your phone (preferred) or the Charm patient portal on your computer.

Ozone Therapy has been around for approximately 150 years. Medical-grade ozone therapy involves using ozone, a gas made up of 3 oxygen molecules, for therapeutic purposes. It has the potential for several health benefits, including improved oxygenation, immune system support, and potential antimicrobial effects. There are hundreds and thousands of promising published literature on the safety and efficacy of ozone therapy. Overall efficacy may vary from person to person. Our staff at Roots has been properly trained and certified in the administration of ozone therapy.

IV Nutrient Therapy involves administering vitamins, minerals, antioxidants, and other nutrients directly into the bloodstream through an intravenous drip. High-dose vitamin C, glutathione, and B-complex are often included. There are several potential health benefits, such as enhanced energy, immune system support, and antioxidant effects. Overall efficacy may vary from person to person.

Yes! Either GLP-1 (semaglutide) or GIP/GLP-1 (tirzepatide) weight loss injections are available for the Provider to order and administer at Roots Integrative Medicine. There are certain safety procedures that will take place before the Provider orders a weight loss injection, to include a consultation to review health history, potential risks and side effects, review of lab work, etc. Once informed consent and safety procedures are obtained, the Provider will send an order for the medication to be compounded by a specialized pharmacy and then administered at Roots.
