Service Description

Wellness Visits/Yearly Physicals for when you/your child are overall well. If during a Wellness Visit, the Provider addresses more than 1 significant medical issue with you/your child, then you/your child may be subject to a Wellness Visit charge, PLUS a Follow Up Visit charge. See below for examples: 1) Wellness Visit: -Patient is overall well -Up to 1 medical issue can be discussed during the Wellness Visit (i.e., concerns around a rash) -This would be a $99 Wellness Visit 2) Wellness + Follow Up Visit Charge: -Patient is overall well OR Patient is sick during wellness visit requiring workup of sick symptoms (this will typically exclude common cold symptoms/other minor symptoms) -2+ medical issues discussed during the Wellness Visit (i.e., concerns around a rash PLUS trouble sleeping) -This would be a $99 Wellness Visit plus a Follow Up Visit charge Well Visits/Yearly Physicals may include: EKG, Pap, urinalysis, thorough intake/exam, anxiety/depression screenings, & venipuncture (blood draw) costs for certain patient populations and is included in the $99 base price. If a patient denies one of these services during a Well Visit or if it is not applicable to them during a Well Visit, the price still stays at $99. Any other procedures or additional time spent during a Well Visit will be billed accordingly and in addition to the $99 base price.
